What do the AHA categories actually classify?

The labels these apps display come from the American Heart Association's published boundaries. We run a free blood pressure category checker on this site and publish the exact cut-offs it applies, in millimetres of mercury:

  • Hypertensive Crisis: systolic 180 or above, or diastolic 120 or above.
  • High Blood Pressure (Stage 2): systolic 140 or above, or diastolic 90 or above.
  • High Blood Pressure (Stage 1): systolic 130 or above, or diastolic 80 or above.
  • Elevated: systolic 120 to 129, with diastolic below 80.
  • Normal: anything below those lines.

The part worth understanding is the order in which they are applied. The bands are tested from the most severe downwards and the first one that matches is the one reported. That single rule produces the behaviour people find surprising: a reading of 122/82 is reported as Stage 1, because the lower number has already crossed the Stage 1 line even though the upper number on its own would read as Elevated. Whichever of the two figures sits in the worse band decides the result.

It is worth checking that rule in whichever app you install, because an app that classifies on the systolic figure alone will label that same reading differently. Why does the log matter more than any single reading?

A reading is one point. A log is a series, and a series is the only thing an app can add that a monitor cannot. Most free apps record the two numbers and stop there, which is enough for a display and not enough for a record. Where do your readings end up?

A blood pressure history is health data, and where it is stored is a decision the app makes on your behalf on the first launch. An app that requires an account has, by construction, put your readings on a server you do not control. An offline app has not, which also means there is no account to lose access to and no sync to break when you change phones.

The practical check takes a minute: turn off mobile data and Wi-Fi, then open the app and add a reading. If it will not record one, the log was never really yours to begin with.
